Active&Fit Direct

The Active&Fit Direct program is a low-cost fitness program available to you and your spouse or domestic partner when enrolled in a medical plan. Your membership includes a gym membership to your choice of 11,600+ standard gyms for $25/month and/or 5,000+ premium exercise studios with 20%-70% discounts on most. You can switch gyms or cancel any time. You will also have access to over 7,600 on-demand workout videos so you can work out at home or on the go.

Log in to www.anthem.com, chose Care, and select Discounts.

WINFertility

WINFertility infertility management provides support and resources for those trying to become pregnant. You must be enrolled in a medical plan to access the program. As a USACS employee, you will receive 24/7 access to a WINFertility nurse care manager and referrals to high-quality reproductive endocrinologists and other specialists from the exclusive provider network, as well as lower than market-rate treatment bundles. This program has options for treatment and medications or medication only at discounts of 30-40% off retail costs.

To enroll or learn more:

1. Log in to www.anthem.com, choose the Care tab, then select Discounts.
2. Select the WINFertility special offer. (If it does not appear, select Load More Offers.)
3. The WINFertility site will list a toll-free number and a promo code to enter to redeem the special offer.
