Eligibility
US Acute Care Solutions offers a variety of benefits to support your and your family’s needs. Choose options based on what’s important to your unique lifestyle.
If you are a full-time employee of USACS, classified as a clinical employee, and working at least 108 hours per month, or a non-clinical employee working at least 25 hours per week, you are eligible to participate in the medical, dental, vision, life, and disability plans, and additional benefits.
How to Enroll
Enrollment will be in Workday. Access your enrollment task via your Workday inbox during your enrollment window. Mobile access is also available on the Workday app. The elections you make during the annual Open Enrollment period become effective January 1 each year. If you are a new hire, your elections are effective the first day of active employment. You won’t be able to change your benefits until the next enrollment period unless you experience a Qualified Life Event.
Eligible Dependents
Dependents eligible for coverage include:
- Your legal spouse or domestic partner.*
- Children up to age 26.
- Dependent children 26 or more years old who are incapable of self-care.
*If you cover a domestic partner, your premiums will be paid on an after-tax basis. Based on IRS guidelines, you will be required to pay taxes on the value of the benefits paid for by the Company for your domestic partner coverage.
Verification of dependent eligibility may be required upon enrollment.
Ensure the dependents you’re enrolling are eligible for coverage. Here are some commonly added dependents who may not be eligible:
- Ex-wives and ex-husbands
- Former stepchildren
- Nieces and nephews
- Grandchildren
- Employee’s parents
- Adult children
If you realize that you are covering an ineligible dependent, you may remove that dependent from your insurance with no questions asked.
Working Spouse Surcharge
If your spouse has access to healthcare coverage through another employer and elects coverage through USACS, they are subject to a monthly surcharge. The surcharge does not apply if your spouse does not work, works part time, is not eligible for coverage, has lost coverage as an active employee but has been offered COBRA, or is covered by Medicare.
Note: The Company reserves the right to verify if your spouse is provided coverage elsewhere. Misrepresenting whether your spouse has access to medical coverage may result in disciplinary action.
